Fiosrú Witness appeal: Incident on Saturday 28 March 2026 in the Westmeath / Meath Division

Fiosrú, the Office of the Police Ombudsman, is independently investigating the sudden death of a man on Saturday, 28 March 2026 in Mullingar, Co. Westmeath.
The referral was made by a Garda Superintendent under section 203(1) of the Policing, Security and Community Safety Act, 2024.
Fiosrú is appealing to any witnesses to this incident to come forward.
Fiosrú would like to hear from anyone who was on the Main Street, Mullingar, at approximately 10.00 – 11.00am on Saturday, 28 March 2026. We would also like to receive dashcam footage from passing vehicles from that location and /or mobile device footage from anyone who may have witnessed the man and his interaction with gardaí.

The referral was made by An Garda Síochána, following the death of a man in the Westmeath / Meath Division.
One of Fiosrú’s on call team of investigators attended the scene.
The team have and will continue to engage with the family concerned.
As the matter is now under investigation, pursuant to section 208(1)(b) of the Policing, Security and Community Safety Act, 2024, Fiosrú cannot make any further comment.
Section 203(1) of the Policing, Security and Community Safety Act, 2024 provides for the referral to Fiosrú any matter concerning death of, or serious harm to, a person.
‘The Garda Commissioner shall, subject to subsection (2), without delay, refer to the Police Ombudsman any matter that appears to the Garda Commissioner to indicate that the act or omission of a member of garda personnel may have resulted in the death of, or serious harm to, a person.’
